Rehabilitation Referrals
Referrals for rehabilitation services can be submitted via fax to 08 8377 0543 or through our eReferrals portal (Click the Referrer Login button below).
To use the portal, you’ll need a valid email address and contact phone number for registration.
Once your referral is received, the team at Griffith Rehabilitation Hospital will arrange an assessment for your patient.
Essential Information for All Referrals
To ensure timely assessment and admission, please include:
- Patient name and date of birth
- Diagnosis details
- Referring doctor details
- Requested admission/transfer date and assessment date
- Funding details (e.g., private health insurance, Department of Veterans’ Affairs, workers compensation, etc.)

Day Rehabilitation Referral
Day rehabilitation referrals can be faxed to 08 8298 5520 or submitted online using the referral form available through the link below.
